Care Coordinators

Primary Digital Health Care coordinators are experts at facilitating appointments with our providers for members who wish to be seen for a primary care visit. Once a primary care visit is requested, the care coordinator will schedule the member at the next available appointment time with one of our providers who participates in the primary care program. The provider will communicate with the care coordinator before, during, and after the visit to facilitate ongoing care management.

In addition to these conditions, the providers will work with members on preventive techniques, medication
management, treatment goals, and long-term follow-up. This will allow them to continue their journey of staying healthy and happy knowing that they have a provider team who is committed to their overall well-being.

New Member Visit – How to get started

During the new member visit, the Primary Digital Health primary care provider will spend up to 45 minutes with the member to understand their full medical history and background to provide them with the best care possible. This will involve taking a comprehensive history including social and family history, prior screening and immunization status, a virtual physical exam, and development of an ongoing plan for the initial and future needs of the member. During this visit, the provider will order any labs or diagnostics that are needed based on the needs of the member as well as order any immunizations or medications that the member may need. In addition to addressing any acute complaints, the provider will also encourage long-term wellness activities and create a future plan with the member.

If the member already has a primary care provider (PCP), they can still use our service to fill any gaps in their care including co-management of chronic disease, treating acute illness due to long-wait times with their PCP, or facilitating the coordination of screening tests and immunizations. If the member does not have a local PCP, our provider will become the member’s PCP and follow this member begin a long-standing relationship and develop a whole-person approach to their health.

Convenient Mental Health Counseling Support – $50 Service Fee

(Enhanced Membership Only)

We provide a safe, secure, and private means of seeking mental health assistance from licensed mental health professionals via virtual or telephonic counseling sessions. Virtual and telephonic counseling options make it easy to access mental wellness. Support is available seven days a week, and some options are available 24/7. Support options promote mental wellness, contributing to a happier, more productive workforce.

Virtual Mental Health Options

Mental wellness: Schedule appointments with a licensed mental health professional seven days a week from 7 a.m. to 10 p.m.
Smart virtual care: Access behavioral health screening tools, assessments and treatment plans with medical healthcare providers.
Chatbot: The AI-powered interactive chatbot built by psychologists is available 24/7 and specializes in anxiety and depression.

How Does Primary Digital Health Work?

  1. Identify the body part needing treatment.
  2. Answer a few short questions about the injury.
  3. Immediately receive a research-based and condition-matched exercise recovery plan, utilizing professionally planned video exercises which can be adjusted as needed.
  4. Attend a virtual follow up visit with a physical therapist for recovery review and referral to in-network clinician, if necessary, upon completion of the 21-day recovery plan.

Virtual First Physical Therapy for Injury Recovery

AFE members who have muscle and joint pain never receive the care they need. This can lead to unnecessary injections, imaging, invasive surgery, workers’ compensation claims, and potential opioid addiction, leading to costs that are extremely high. To counter this, with clinical oversight prior to beginning a treatment regimen, our team of physical therapists created our case management tool that uses specific exercises to address injuries. The case management tool uses exercise videos in full sets, played in real time, and narrated by a physical therapist, all on a 24/7, self-service website.

Primary Digital Health allows AFE members to access help on their own schedule, rather than going to a traditional clinic, allowing them to address any issues that arise before they start to significantly impact their well-being.

Reduce Sick Days

The overall incidence rate of non-fatal occupational injury and illness cases requiring time away from work is a median of 8 days per year. By making Primary Digital Health available, AFE members can be proactive about their physical well-being.

Increase Access to Care

Many people who seek professional help for muscle and joint pain fail to complete their course of treatment due to geographical constraints, financial concerns, or the inability to attend treatment during work hours. With Primary Digital Health, AFE members can access care any time, any place. By translating complex medical terms into easy-to-understand vocabulary, we reduce the barriers to entry, enabling easy access for all.
